The following forms are available in Adobe Reader and Microsoft Word. Please note: not all forms are available in both formats. For Adobe Reader you will need Version 8, which is the latest version. You can download Adobe Reader free at www.adobe.com. For the Word format, you will be able to fill it in, print, and save the form. Please remember to change the name of the file (using "Save As") or you will lose the original form. You can then email these forms. For the Adobe Reader format (which we recommend) you can fill it in and print it, but you will be unable to save your changes. You must either scan your printed copy and then email it or fax or mail a copy to the District Office.

PASTORS, the Annual Pastor's Report is only a worksheet for you to print and use when filling out your report online. The Finance Instructions will assist you in filling out this worksheet. To submit your report, you must go to www.apr.nazarene.org. If you do not know your password, please contact the District Office.
